Edit Child Details or Change The Answers - Parent Portal

You can log in using the link that your school sent you. If you have already registered, you can log in at portal.schoolscreener.com 

After logging into the portal, please click Forms -> Submitted -> View Submission.

If the health team allows further amendments to the form and the programme is still active, you will be able to make the changes as necessary.

Here you can edit the details, then click the 'Continue' button.

Amend any of your answers if you need to on the next page and click "Submit".

The confirmation of submission will be sent to your email. Please check your spam folder if it hasn't reached your main inbox within 10 minutes.
