How to Add a Form for Another Child

These instructions apply only if the other child has been invited to take part in the same health programme at the same school.

If you need to complete a form for another child, follow these simple steps:

Step 1: Select 'Fill in for Another Child'

• On the form you’ve already submitted, click ‘Fill in for another child’.

• A dialogue box will appear – click ‘Yes’ and follow the on-screen instructions.

Step 2: Start a New Submission

• A new form will be created under the New section.

• Click ‘Start submission’ and enter your other child’s details.

Step 3: If Your Child Attends a Different School or Programme

• If your second child is in a different school or is part of a different screening or immunisation programme, you’ll need a separate invitation link.
• Contact the school and ask them to send you the correct link.
• Instead of registering again, simply log in using that link to complete the form.
