How do I start filling in the form?

Getting started with your form is quick and easy! Just follow these steps:

1. Find Your Form

Once you’ve logged into your account, you’ll see your account dashboard. Any forms you’ve accessed via a link will be in one of the following tabs:

📌 New – Forms that haven’t been started yet.
📌 In Progress – Forms that you’ve started but haven’t submitted.
📌 Submitted – Forms that have been completed and submitted.



If you’ve just clicked on a new link, the form will appear in the ‘New’ tab.

Click the ‘Start’ button to begin.



2. Confirm Child and School Details

  • Check that the school name and campaign details are correct.
  • Then, either add a new child or select an existing child.


  • Fill in the details for a new child, or check that the information for an existing child is accurate.
  • Click ‘Continue’ to move on.


3. Complete the Form

  • Fill in all the required questions.
  • Once finished, click ‘Submit’ to send the form to the school health team.

  • You will receive a confirmation email letting you know the form was successfully submitted. If you don’t receive the email, please check your spam or junk folder.​
