Adding Another Child to a Form

If your children attend the same school and are invited to take part in the same health programmes (e.g., twins), you can create a new form for them.

However, if your child attends a different school or is due to take part in a different screening or immunisation programme, please ask the school to send you the relevant invitation link. Instead of registering again, simply log in via that link to access the correct form.

How to Add Another Child to a Form

  1. Click 'Fill in for another child' on the respective form.

      2. A dialogue box will appear—click 'Yes' and follow the on-screen instructions.

    3. This will create a new form under the 'New' section.

   4. Select 'Start submission' and enter your other child's details to complete the process.
