What if I submitted the form for the wrong school?
If your child is due to take part in the health programme at a different school, please contact the School Nursing Service as soon as possible.
If you are yet to submit the form, you have two options to address the issue with the wrong school details on your child's form:
- Submit the form with the wrong school and inform the School Nursing Service, who can correct the school name on the submitted form from their end.
- Start a new form for the correct school and submit it. Please note that the original form (with the wrong school name) cannot be deleted.
The contact details for the School Nursing Service are usually displayed on the original invitation letter you received from your child's school.
