If you do not understand one of the questions in the questionnaire or consent form, we suggest you contact the relevant health team.

You will find their contact information in the email sent by your child's school.

We also have contact information available. Please email us: parentsupport@schoolscreener.com with the following information:

  • The name of the school 

  • The town or city where the school is located

  • Child's year group

  • The name of the health programme you’re being asked to sign a form for (e.g., Vision, Growth, Immunisations, etc.)

  • If possible, please attach a copy of the letter you received from the school
