I am unable to submit the questionnaire.

You are only able to submit the questionnaire if you have answered all the mandatory questions. Look back through the questionnaire and see if any of the sections are highlighted (see image below). Once you have answered the question you should be able to click the 'Submit' button at the end of the questionnaire.

         

If you cannot see the "Start/Resume submission" button, this means that the School Nursing Service has closed the link for further submissions.

Please reach out to the School Nursing Service directly to find out more about the available alternatives. Their contact details can be found on the invitation letter you received from the school.
