Do I get a copy of my consent form?

Once you have submitted the consent you will receive an email to let you know we have received it. Please check your spam/junk folder if you don't receive it.

If you have been asked to Register/Log in as part of the consent process, you can always log back into your parent account to view a copy of your answered consent form.
